To request a screening, please complete and sign the Screening Permission Form by clicking the link above. Your child’s teacher may be asked to complete a checklist to provide insights into your child’s classroom performance. When possible, a brief consultation with the teacher will be included in the screening process. A summary of the results will be sent home with your child’s belongings.
If your child is already receiving therapy at another clinic, we are unable to perform a screening as this may conflict with their already established progress.
Occupational Therapy Screening
An Occupational Therapy Screening offers a preliminary assessment of your child’s development to determine if a comprehensive Occupational Therapy Evaluation is necessary. This screening evaluates skills such as sensory processing, attention, and visual/fine motor abilities (e.g., self-care tasks like dressing, feeding, pencil use, grip, and scissor/drawing skills).
Speech Therapy Screening
A Speech Therapy Screening provides an initial measurement of your child’s communication development to ascertain the need for a full Speech Therapy Evaluation. This screening assesses language comprehension, verbal communication skills, and speech intelligibility. Additionally, it can address feeding and oral motor concerns.
Physical Therapy Screening
A Physical Therapy Screening offers a brief review of your child’s gross motor skills to determine if a comprehensive Physical Therapy Evaluation is required. This screening evaluates gross motor abilities, balance, and coordination skills (e.g., rolling/crawling, walking, jumping, running).
